Disability certificate (assessment for persons with disabilities)

Persons with disabilities are assessed by District/Tehsil Disability Assessment Boards at DHQ/THQ hospitals, which issue a disability certificate used for the special CNIC, job quota, Himmat Card and other benefits. Applications are made online through the Disabled Persons Management Information System (DPMIS).

  1. Register and apply on dpmis.punjab.gov.pk
  2. Or visit the Medical Social Officer at the DHQ/THQ hospital
  3. Appear before the Disability Assessment Board (meets fortnightly)
  4. Receive the disability certificate
