Birth registration & computerised birth certificate

Births are registered at the Union Council or Municipal Committee and recorded in NADRA's Civil Registration Management System, producing a computerised birth certificate needed for Form-B, school admission and passports. Under the Punjab Birth & Death Registration Rules 2025, registration within seven years is free and no court decree is needed for late registration.

Who can apply

  • Parents/guardians of a child born in Punjab
  • Event registered within 7 years for free registration

How to apply

  1. Visit the Union Council/Municipal Committee secretary, an e-Khidmat Markaz or use the CRMS app
  2. Fill the birth registration form with parents' details
  3. Submit documents; entry is made in NADRA CRMS
  4. Collect the computerised birth certificate
  5. For a Dastak doorstep option, confirm current availability and instructions in the Dastak Citizen App.

  • Hospital birth slip or proof of birth (if available)
  • Parents' marriage certificate (often requested)
