How to apply
- Ensure family records are updated with NADRA; your original CNIC serves directly as the health card (no separate card needed)
- Check empaneled hospital list at phimc.punjab.gov.pk or call 0800-07542 (24/7)
- Present original CNIC (or B-Form for minors) at the Sehat Sahulat counter in the participating hospital for admission verification
